POST RENAL AKI

A 55 yr old female labour by occupation came with Chief complaints of

Pedal oedema 2 months ago and 
Decreased urine output for one day

History of presenting illness
Patient was apparently asymptomatic 2 months back,then developed pedal oedema in right leg,which is pitting type,and decreased urine output,she had elevated creatinine  for which she kept under dialysis approximatley 10 times
She diagnosed with Renal calculi 2 months back
No history of shortness of breath ,fever,body pains

Past history: had a history of fever for which she went to hospital ,serum creatinine elevated ,she undergone dialysis,and then diagnosed with OBSTUCTIVE UROPATHY for which DJ stenting was done ,Known case of hypertension since 3 yrs
No history of DM ,Asthma ,TB,thyroid and cadiovascular disorders

Family history :
Not significant
GENERAL EXAMINATION:
Patient was ,coherent ,conscious ,cooperative
No pallor,icterus,clubbing,cyanosis,lymphadenopathy, oedema

VITALS
Temp-Afebrile
BP: 130 /80 mmHGg
RR: 20 cpm
PR :80 bpm
GRBS: 105 mg/dl
Systemic examination:
RS: vesicular breath sounds heard
Cvs: s1,s2 heard,no murmers
Abdomen: soft non tender
CNS:  NAD
INVESTIGATIONS:
Ultrasound
